Big release with lots of fixes and improvements.

• Keep screen on while watching streams
• Increase the amount of time player controls stay visible (1s -> 2s)
• Added playback server selector to main screen (doesn't save yet)
• Added loading/processing state to login screen and buttons that needed it
• Added information, confirmation and error dialogs
• Fixed jarring transition from Login page to Online Streams page
• Added pull-down-to-refresh functionality to Online Streams page
• Added custom splash screen and app icon
• "Fixed" chat sometimes not scrolling all the way down
• Chat text box now shows connecting status and guest info
• Chat send button is now hidden when unable to chat (connecting, etc)
• Chat no longer attempts to scroll down when it can't
